ELECTRONIC STATEMENT ELECTION AGREEMENT AND DISCLOSURE
By enrolling in SafeAmerica Credit Union’s Online Banking service, or, for existing Online Banking service enrollees, by logging on/signing in to Online Banking after August 1, 2005, I agree that I will receive electronically all disclosures, statements and notices that SafeAmerica is legally required to send to members or chooses to send to members, unless SafeAmerica, at its option, requires me to receive paper documents in connection with a particular transaction type (such as a real estate loan). I understand that I may elect at any time to receive paper statements and disclosures. My most recent election in the Credit Union’s records will determine whether I receive paper or electronic statements and disclosures. Electronic statements and disclosures will contain all the same information as paper statements and disclosures. I will have the same opportunities to contact the Credit Union about any errors or problems. I confirm that I have a computer with Internet access, an e-mail address, and a password to access Online Banking. In addition, you will need a browser that meets the following requirements: Netscape version 4.7 or higher, Microsoft's Internet Explorer version 4.0 or higher, or AOL version 5.0 or higher and must have JavaScript enabled. For security reasons we use 128-bit encryption, for this reason you must also use a 128-bit encryption capable version of the browsers listed above. Periodic account and loan e-statements and other disclosures will available for pick up at SafeAmerica Credit Union’s secure web site. SafeAmerica will send me an e-mail confirming that the statements or disclosures are ready to pickup. To access my e-mail statements or disclosures, I will log onto Online Banking at www.safeamerica.com using my PIN. Once I have logged on successfully, I select the “e-statements” or “e-disclosures” button on the navigation strip. E-statements will be available in Online Banking and will accrue to a total of 18 months. I will be able to download and print copies of e-statements if I wish to do so. To download and print statements, I will need a recent version of the above mentioned browsers and a functioning printer. I agree to keep SafeAmerica informed of any change in my e-mail address. If SafeAmerica attempts to e-mail notices to me and SafeAmerica’s e-mail is returned because my address is bad, SafeAmerica will attempt to contact me by phone or postal mail to obtain a valid e-mail address. If SafeAmerica cannot obtain a valid e-mail address from me, SafeAmerica will resume sending paper statements, disclosures and other notices to the last valid address SafeAmerica has on record for me, and charge me any paper statement fee stated in the SafeAmerica Fee Schedule. If I later decide that I prefer to have paper statements mailed to me, I can submit a secure online request through Online Banking to reinstate paper statements and discontinue e-statements, or I can submit my request by mail or in person at any SafeAmerica Credit Union office. Paper statements will be subject to any fee stated in the SafeAmerica Fee Schedule. If any of my accounts or loans are joint, SafeAmerica will honor the instructions of any joint owner or borrower. However, if SafeAmerica receives conflicting instructions, it can send me paper statements and disclosures and charge me any paper statement fee stated on the SafeAmerica Fee Schedule.
